Why do I need to fill out this form?
Applications to the program will close on 21st January 2011 so we can conduct a full audit. We want to be sure that everyone registered at the World Pension Plus Site is also registered on the Yardas Data Base. The information on the form below is the first stage of verification of your membership at World Pension Plus, so be sure to complete it correctly.

Please insert your details below EXACTLY as listed on the WPP site. This information will then be compared with the data received from them to ensure that all members are accounted for and verified as active.

The first line of your address as listed on the World Pension Plus Website.

Address Line 2:

The second line of your address as listed on the World Pension Plus Website.

State/Provence:

The State or Provence if you have one listed in your account details on the World Pension Plus site. This is optional as not everyone has one.

The City, Suburb, Town or Village of your address as listed on the World Pension Plus Website.

Your Postal or Zip Code of your address as listed on the World Pension Plus Website.
